Before you are able to take part in a parachute jump you will need to complete a
'declaration of fitness' issued by the British Parachute Association (BPA). This
form will be sent to you by post with confirmation details of your jump, and it
must be completed and brought with you on the day of the jump. You may be able
to sign the form yourself, but if you suffer from certain medical conditions, or
are aged 40 or over, you will need to have the form signed by your doctor.
